GoHealth Urgent Care and its U.S. affiliates and subsidiaries (collectively, "GoHealth") respect your concerns about privacy. This U.S. Privacy Policy (the "Policy") describes the types of personal information we collect, how we use the information, with whom we may share it, and the choices available to you regarding our use of the information. We also describe measures we take to protect the security of the information and how you can contact us about our privacy practices.

This Policy applies to personal information collected through our website ("Site") and mobile application ("App") (collectively, "Digital Properties") except where a service has a separate privacy notice that does not incorporate this Policy.

This Policy does not apply to Protected Health Information. Please see our Notice of Privacy Practices for information about how medical information about you may be used and disclosed and how you can access that information.

Information We Obtain

The types of personal information we may obtain when you visit our Digital Properties include:

 Contact information, such as name, telephone number, postal address, and email address;

  • Identifying information such as date of birth;
  • Payment information, such as name, billing address, and payment card details (including card number, expiration date and security code);
  • Social media information, such as social media handles, content and other data shared with us through social media pages (such as our social media pages or other pages accessible to us);
  • Geolocation information, with your consent;
  • Information you provide in the course of applying for a job with us, such as details about your education, work and military history, legal work eligibility status, and background check information; and
  • Other information you choose to provide, such as through our "Contact Us" feature, such as emails or other customer support tools.

If you are using our App, you also may choose to provide the App with permission to access and use your device’s calendar, camera, photos and audio recordings.

When you visit our Site, we may obtain certain information by automated means, such as cookies, web beacons, web server logs and other technologies. A "cookie" is a text file that websites send to a visitor’s computer or other internet-connected device to uniquely identify the visitor’s browser or to store information or settings in the browser. A "web beacon," also known as an internet tag, pixel tag or clear GIF, links web pages to web servers and cookies and may be used to transmit information collected through cookies back to a web server. The information we collect in this manner may include your device IP address, unique device identifier, web browser characteristics, device characteristics, operating system, language preferences, referring URLs, clickstream data, and dates and times of website visits. 

When you use our App, we also may collect certain information by automated means, such as through device logs, server logs and other technologies. The information we collect in this manner may include the device type used, the mobile operating system, device identifiers and similar unique identifiers, device settings and configurations, IP addresses, battery and signal strength, usage statistics, referring emails and web addresses, dates and times of usage, actions taken on the App, and other information regarding use of the App. 

We may use these automated technologies on our Digital Properties to collect information about your equipment, browsing actions, and usage patterns. These technologies help us (1) remember your information so you do not have to re-enter it; (2) track and understand how you use and interact with our Digital Properties; (3) tailor the Digital Properties around your preferences; (4) measure the usability of our Digital Properties and the effectiveness of our communications; and (5) otherwise manage and enhance our services, and help ensure they are working properly. 

In addition, as indicated above, we may collect your device’s geolocation information. With your permission, we may receive the precise geolocation and other location information of your device through GPS, Bluetooth signals, WiFi network identifiers and signals, and other technologies. Your device may provide you with a notification when the Digital Properties attempt to collect your precise geolocation. Please note that if you decline to allow the Digital Properties to collect your precise geolocation, you may not be able to use all the Digital Properties’ features or the offers available through the Digital Properties. 

Your browser may tell you how to be notified about certain types of automated collection technologies and how to restrict or disable them. Please note, however, that without these technologies, you may not be able to use all the features of our Digital Properties. For mobile devices, you can manage how your device and browser share certain device data by adjusting the privacy and security settings on your mobile device. 

How We Use the Information We Obtain

We may use the personal information we obtain to:

  • Provide our services; 
  • Process and fulfill transactions in connection with our services; 
  • Communicate with you (including providing you with offers and other communications about our services), respond to inquiries and offer customer support; 
  • Verify your identity; 
  • Personalize your experience on our Digital Properties; 
  • Advertise and market our services; 
  • Administer participation in surveys, sweepstakes, promotions or other programs; 
  • Respond to your requests and inquiries and provide customer support; 
  • Manage career opportunities with us, including for recruitment purposes, candidate screening and evaluation, and employee onboarding;

  • Maintain and enhance the safety and security of our Digital Properties and services and prevent misuse; 
  • Exercise our rights and remedies and defend against legal claims; 
  • Protect against, identify and prevent fraud and other criminal activity, claims and other liabilities; and
  • Comply with and enforce applicable legal requirements, relevant industry standards and GoHealth policies, including our Terms of Use.

We also may use the information in other ways for which we provide specific notice at the time of collection.

Third-Party Analytics Services

We may use third-party analytics services on our Digital Properties, such as Google Analytics. The information we obtain through the Digital Properties may be disclosed to or collected directly by these services. Interest-Based Advertising

On our Digital Properties, we may obtain information about your online activities to provide you with advertising about services that may be tailored to your interests. This section of our Policy provides details and explains how to exercise certain choices. 

You may see our ads on other websites because we use third-party ad services. Through these ad services, we can target our messaging to users considering demographic data, users’ inferred interests and browsing context. These services track your online activities over time and across multiple websites and apps by collecting information through automated means, including through the use of cookies, web server logs, web beacons and other similar technologies. The ad services use this information to show you ads that may be tailored to your individual interests. The information ad services may collect includes data about your visits to websites that serve GoHealth advertisements, such as the pages or ads you view and the actions you take on the websites or apps. This data collection takes place both on our Digital Properties and on third-party websites and apps that participate in these ad services. This process also helps us track the effectiveness of our marketing efforts. 

The Digital Properties are not designed to respond to "do not track" signals from browsers.

Information We Share

We may share the information we obtain about you with our affiliates and subsidiaries and with our joint marketing partners. We also may share the information we obtain about you with third-party vendors and other entities to perform services on our behalf, such as website hosting, data analytics, advertising, payment processing and other services. 

We also may disclose personal information (1) if we are required to do so by law or legal process (such as a court order or subpoena); (2) in response to requests by government agencies, such as law enforcement authorities; (3) to establish, exercise or defend our legal rights; (4) when we believe disclosure is necessary or appropriate to prevent physical or other harm or financial loss; (5) in connection with an investigation of suspected or actual illegal activity or (6) otherwise with your consent or as directed by your representative. 

We reserve the right to transfer any personal information we have about you in the event we sell or transfer all or a portion of our business or assets (including in the event of a merger, acquisition, joint venture, reorganization, divestiture, dissolution or liquidation).

How We Protect Personal Information

We maintain administrative, technical, and physical safeguards designed to protect the personal information you provide against accidental, unlawful, or unauthorized access, destruction, loss, alteration, disclosure or use. 

Children’s Privacy

Our Digital Properties are designed for a general audience and are not directed to children. In connection with the Digital Properties, we do not knowingly solicit or collect personal information from children under the age of 13 without parental consent. If we learn that we have collected personal information from a child under age 13 without parental consent, we will either seek parental consent or promptly delete that information. If you believe that a child under age 13 may have provided us with personal information without parental consent, please contact us as specified in the How To Contact Us section of this Policy.

For your convenience and information, our Digital Properties may provide links to other online services, and may include third-party features such as apps, tools, widgets and plug-ins. California Consumer Privacy Statement

This California Consumer Privacy Statement (the "Statement") supplements the GoHealth Urgent Care U.S. Privacy Policy. It applies solely to California consumers, including personnel working on behalf of our business partners (e.g., vendors), subject to the exceptions listed below.  For clarity, this Statement applies to personal information we collect about California consumers both online and offline.

This Statement does not apply to:

  • GoHealth Urgent Care personnel or job applicants.
  • Medical information governed by the Confidentiality of Medical Information Act.
  • Protected health information ("PHI") we collect that is governed by the privacy, security, and breach notification rules issued by the U.S. Department of Health and Human Services, Parts 160 and 164 of Title 45 of the Code of Federal Regulations, established pursuant to the Health Insurance Portability and Accountability Act of 1996 (Public Law 104-191) (“HIPAA”) and the Health Information Technology for Economic and Clinical Health Act (Public Law 111-5) (“HITECH Act”).

We may use (and may have used during the 12-month period prior to the effective date of this Statement) your personal information for the purposes described in our Privacy Policy and for the following business purposes specified in the CCPA/CPRA:

  • Performing services, including maintaining or servicing accounts, providing customer service, processing or fulfilling orders and transactions, verifying customer information, processing payments, providing financing, providing analytics services, providing storage, or providing similar services
  • Providing advertising and marketing services to you, except for cross-context behavioral advertising (which is addressed in the “Sale and Sharing of Personal Information” section of this Statement)
  • Auditing related to counting ad impressions to unique visitors, verifying positioning and quality of ad impressions, and auditing compliance
  • Short-term, transient use, including, but not limited to, non-personalized advertising shown as part of your current interaction with us;

We do not collect or process sensitive personal information with the purpose of inferring characteristics about California consumers covered by this Statement.

We will retain your personal information for the period reasonably necessary to achieve the purposes described in our Privacy Policy and this Statement, or any other notice provided at the time of collection, taking into account applicable statutes of limitation and records retention requirements under applicable law.

Under California law, these kinds of disclosures may be considered a “sale” when the personal information is exchanged for non-monetary consideration, or “sharing” when the personal information is disclosed for cross-context behavioral advertising purposes. You have the right to opt out of these types of disclosures of your information.

Verifying Requests. To help protect your privacy and maintain security, we will take steps to verify your identity before complying with your request.  If you request access to, correction of or deletion of your personal information, we may require you to verify your email address or certain of your information in our records, such as your name, email address, mailing address, phone number, company name and job-details, or other information about your interactions with us.  In addition, if you ask us to provide you with specific pieces of personal information, we may require you to sign a declaration under penalty of perjury that you are the consumer whose personal information is the subject of the request.

If you do not have an email address or other direct contact information on file with us, then there is no reasonable method by which we can verify your identity to the level of certainty required by the CCPA/CPRA.  The reason for this is that we historically have not linked IP addresses, device identifiers or other information collected through automated means to named actual persons. Accordingly, if you do not have an email address or other direct contact information on file with us, and you request access to, correction of or deletion of your personal information, we will not be able to process your request at this time.

Additional Information. If you choose to exercise any of your rights under the CCPA/CPRA, you have the right to not receive discriminatory treatment by us. To the extent permitted by applicable law, we may charge a reasonable fee to comply with your request.

It applies solely to California consumers who are job applicants to UCSF Health-GoHealth Urgent Care Management, LLC or to GoHealth Urgent Care (“CA Job Applicants”) and addresses personal information we collect about them both online and offline. This Statement does not apply to personnel who do not live in California.

Verifying Requests. To help protect your privacy and maintain security, we will take steps to verify your identity before granting you access to your personal information or complying with your request. We may verify your identity by requiring you to sign in to your Job Application Account. If you do not have an account and you request access to, correction of or deletion of your personal information, we may require you to provide any of the following information: first and last name and email address. In addition, if you do not have an account and you ask us to provide you with specific pieces of personal information, we will require you to sign a declaration under penalty of perjury that you are the CA Job Applicant whose personal information is the subject of the request.
